परिचय

बर्न्स और प्लास्टिक सर्जरी विभाग निकट और दूर से आने वाले सभी वंचित रोगियों को अत्यंत समर्पण और खानपान के साथ व्यापक रोगी देखभाल प्रदान करने के लिए प्रतिबद्ध है। विभाग अपनी व्यापक नियमित सर्जरी के अलावा ट्रॉमा और इमरजेंसी, न्यूरोसर्जरी, कार्डियक सर्जरी, आर्थोपेडिक सर्जरी, ईएनटी, दंत चिकित्सा आदि जैसे कई अन्य विभागों के सहयोग से काम करता है। सर्जनों की उत्साही और उत्साही टीम की इस सहयोगी टीम का महत्वाकांक्षी लक्ष्य रुग्णता को कम करना और उन्नत माइक्रोसर्जिकल तकनीकों सहित पुनर्निर्माण प्रक्रियाओं द्वारा कार्यक्षमता प्रदान करना है जिससे रोगियों को जीवन की बेहतर गुणवत्ता प्रदान की जा सके। यह लगभग सभी आपातकालीन हाथ की चोटों, जटिल अंग आघात के लिए माइक्रोवस्कुलर पुनर्निर्माण, जलने के बाद पुनर्निर्माण, मैक्सिलोफेशियल आघात, ऑन्कोप्लास्टिक पुनर्निर्माण, ऑन्कोप्लास्टिक स्तन पुनर्निर्माण और अन्य जन्मजात विसंगतियों जैसे कटे होंठ, कटे तालु, जन्मजात हाथ की विकृति (सिंडैक्टली, पॉलीडेक्टाइली, कैंपटोडैक्टली, हाइपोप्लास्टिक अंगूठे की विकृति, आदि) को पूरा करता है। नकारात्मक दबाव घाव चिकित्सा प्रणाली (एनपीडब्ल्यूटी) का उपयोग करके उन्नत घाव प्रबंधन और यहां तक कि प्रदान भी करता है। एस्थेटिक ब्रेस्ट सर्जरी, एस्थेटिक फेशियल सर्जरी और गाइनेकोमेस्टिया, एब्डोमिनोप्लास्टी, ब्रैकियोप्लास्टी, थाईप्लास्टी और टोटल बॉडी कॉन्टूरिंग प्रक्रियाओं आदि सहित सौंदर्य देखभाल में उत्कृष्टता। विभाग को माइक्रोवस्कुलर और ब्रेकियल प्लेक्सस रिकंस्ट्रक्टिव सर्जरी में अग्रणी काम के लिए पूरे राज्य में मान्यता प्राप्त है।

विभाग में चार संकाय सदस्य, 1 अतिरिक्त प्रोफेसर, 2 एसोसिएट प्रोफेसर और 1 सहायक प्रोफेसर हैं। संकाय सदस्यों को 6 एम.सी.एच. द्वारा समर्थित किया जाता है। सीनियर रेजिडेंट, 2 जूनियर रेजिडेंट, 1 स्टाफ नर्स, 1 लोअर डिवीजन क्लर्क, 1 ओटी तकनीशियन और 3 हॉस्पिटल अटेंडेंट। एम.सी.एच. (प्लास्टिक एवं रिकंस्ट्रक्टिव सर्जरी) कार्यक्रम जनवरी 2021 सत्र से तीन एम.सी.एच. के वार्षिक प्रवेश के साथ शुरू किया गया है। रहने वाले। वर्तमान में हमारे एम.सीएच कार्यक्रम में 6 वरिष्ठ रेजिडेंट हैं।

विभाग ने अपने लेजर सुइट का संचालन शुरू कर दिया है, जिसमें 2 अच्छी तरह से सुसज्जित कक्ष शामिल हैं, जिसमें एक अत्यधिक उन्नत लेजर मशीन है, जो संवहनी घावों, वर्णक घावों, पुरानी घावों, बालों को हटाने और चेहरे के कायाकल्प सहित पुनर्स्थापनात्मक और सौंदर्य प्रक्रियाओं की एक विस्तृत श्रृंखला को करने के लिए सुसज्जित है।

वर्तमान में, अत्याधुनिक बर्न्स सेंटर का निर्माण कार्य चल रहा है। यह केंद्र आईसीयू, त्वचा बैंक और पुनर्योजी चिकित्सा प्रयोगशाला की सुविधाओं के साथ पूरे बिहार, झारखंड और देश के अन्य पूर्वी राज्यों के जले हुए रोगियों की देखभाल करेगा

वर्ष के मुख्य बिंदु:

विभाग ने कोविड रोगियों की अनुकरणीय देखभाल में अपने पूरे दिल से योगदान के साथ एक समर्पित कोविड अस्पताल (डीसीएच) के रूप में एम्स की घोषणा में सख्ती से भाग लिया।
केवल कुछ सरकारी केंद्रों में उपलब्ध अत्याधुनिक मशीन के साथ पुनर्स्थापनात्मक और सौंदर्य प्रक्रियाओं दोनों के रूप में अपनी लेजर सेवाएं शुरू कीं।
अन्यत्र मानक देखभाल का खर्च वहन करने में सक्षम नहीं होने वाले वंचित रोगियों के लिए अपनी लिपोसक्शन सेवाएं जारी रखना।
माइक्रोवैस्कुलर मुक्त फ्लैप्स में शिरापरक एनास्टोमोसिस के लिए नियमित रूप से वेनस कप्लर्स का उपयोग करना, एक नवीनतम तकनीक जो भारत में केवल 4-5 केंद्रों पर उपलब्ध है।
संस्थान के सामुदायिक आउटरीच कार्यक्रम के तहत सीतामढी (बिहार का जिला) में लाइफलाइन एक्सप्रेस द्वारा आयोजित सर्जिकल शिविरों में संकाय सदस्यों ने प्लास्टिक सर्जरी टीम के लिए संसाधन व्यक्तियों के रूप में भाग लिया।
संकाय सदस्यों ने संस्थान के क्रिटिकल थिंकिंग फाउंडेशन कोर्स (CTFC) कार्यक्रम को सक्रिय रूप से संचालित करने के लिए एक समर्पित टीम में संसाधन व्यक्तियों के रूप में भाग लिया।
संकाय सदस्यों ने ""स्तन कैंसर जागरूकता सप्ताह"" के दौरान भाग लिया और संस्थान के कर्मचारियों के परिवार के सदस्यों के लिए ""स्तन पुनर्निर्माण"" पर सार्वजनिक जागरूकता का आयोजन किया।
संकाय सदस्य व्यापक जागरूकता अभियानों के साथ-साथ स्नातकोत्तर शिक्षण के संचालन के लिए स्पेशलिटीज नेशनल एसोसिएशन ऑफ बर्न्स इंडिया (एनएबीआई) द्वारा आयोजित नियमित ऑनलाइन वेबिनार से जुड़े हुए हैं।
बिहार सरकार के आपदा प्रबंधन विभाग द्वारा आयोजित राष्ट्रीय सड़क सुरक्षा सप्ताह और राष्ट्रीय अग्नि सुरक्षा सप्ताह के दौरान स्वास्थ्य शिविर आयोजित करने और जागरूकता बढ़ाने के लिए एक संकाय सदस्य ने एक संसाधन व्यक्ति के रूप में कार्य किया।

वर्ष 2019-20 में विभाग की एक प्रमुख उपलब्धि भारत सरकार के तहत एक राष्ट्रीय परियोजना राष्ट्रीय बाल स्वास्थ्य कार्यक्रम (आरबीएसके) के तहत कटे होंठ और तालू के रोगियों के लिए निःशुल्क सर्जरी की पेशकश करना रहा है। विभाग के संकाय इसके लिए नोडल अधिकारी हैं और इस कार्यक्रम के तहत शामिल जन्मजात विकृतियों के सर्जिकल प्रबंधन के लिए बाल चिकित्सा सर्जरी, ईएनटी, ऑर्थोपेडिक्स, बाल चिकित्सा, नेत्र विज्ञान और दंत चिकित्सा जैसे अन्य विभागों के साथ समन्वय करते हैं।

आधारभूत संरचना

विभाग और ओपीडी ओपीडी ब्लॉक की 5वीं मंजिल पर स्थित है। वार्ड में, वर्तमान में डी5बी ब्लॉक, आईपीडी बिल्डिंग में बिस्तरों की संख्या 30 है। सेंट्रल ओ.टी. कॉम्प्लेक्स में एक समर्पित ऑपरेशन थियेटर है जो सप्ताह में 5 दिन खुला रहता है।

सेवाएं

Clinical

Although the State being affected by the severe COVID Pandemic the department in association with the institute continued to offer its exceptional services in clinical care to all its patients.

The department is providing Outpatient care from Monday to Saturday dealing in all the aspects of plastic surgery. Specialty clinics of keloid, vascular malformation, hand clinic and cleft clinics are run in the second half on a weekly basis. The department has catered to around 1200 patients in the previous year, the tally being affected by the COVID pandemic

The department has a 30 bedded plastic surgery inpatient ward with dedicated staff for the care of the admitted patients. Total admissions in the last year were 99.

Surgeries are being done daily from Monday to Friday in the newly opened OT complex with 1 OT dedicated to Burns & plastic surgery. Apart from providing elective surgeries, emergency surgeries are also being performed on a need to basis in the trauma OT complex. The department continued specialized surgeries like microvascular reconstructions, brachial plexus surgeries, trauma, hand surgeries, cleft lip & palate surgeries etc. The department has performed over 84major surgeries in the last 3 months of last year. Although the department could not perform to its full potential due to closure between 01/04/2021 to 31/12/2021, as the O.T. services were closed due to AIIMS Patna being declared a 'Dedicated Covid Hospital (DCH)'.

In the last year, newer technology like use of venous couplers for venous anastomosis in microsurgical procedures has started in the department. Additional services like Liposuction have been started for patients undergoing aesthetic procedures.

Non-clinical

The department faculty members participated as members of various committee (Oration committee, Spandan committee, Stage committee) in the Organizing team for Institute Foundation Day celebrated on virtual platform on 25th September 2021. The faculty member has also contributed as member of Editorial team, Annual report of AIIMS Patna.

Teaching and Training

The department is proactively involved in teaching and training activities of undergraduate, postgraduate as well as nursing students including lectures, clinical postings and departmental teachings. The Faculty members are also enlisted as editorial board members, experts of review committees in reputed national and international journals. All the members actively participated in training of medical and police personnel which were organized by institute in collaboration with Dept. of Disaster Management, Govt. of Bihar educating about first aid and CPR.

Future Planning

The future aim of the department is to establish the state-of-the-art Tertiary Burn center at AIIMS which could benefit the hugely deprived population of Bihar. The department also plans to start craniofacial surgeries, ICG Fluoroscopic mapping for intraoperative flap monitoring, Endoscopic plastic surgery. In the training domain, the process for establishing a microsurgical skill lab for developing microsurgical skills of residents and trainees is already in the process.
